Transfer und Umsetzung eines europäischen Präventionskonzeptes am Beispiel eines Gewichtsreduktionsprogramms unter besonderer Berücksichtigung kulturspezifischer Rahmenbedingungen in der Türkei

Abstract

Background:
The problem of overweight and obesity is growing steadily and has reached the magnitude of a worldwide epidemic. To a large extent the people in Turkey are also affected by this problem, for example currently 70.7% of women, with the trend is rising. Sometimes the national efforts are too little or have not done enough to address this problem. As the problem of overweight is a risk factor for the development of various diseases. Medical treatment leads to increased stress on the health system and, most importantly, reduced quality of life, therefore prevention-based lifestyle intervention should be available to all concerned.
In the present study a prevention concept, "The Turbo-Metabolism Principle" developed for German-speaking countries, was tested under the culture-specific conditions in Turkey to determine its practicability and transferability. This concept will offer the overweight and obese persons concerned an evidence-based therapy option for weight loss, and thus to make a significant contribution to international efforts in the fight against overweight and obesity.
Method:
In order to be able to answer the research question in full, the first step was to test the European prevention concept under the culture-specific conditions in the form of nutrition and exercise interventions in Turkey (in Istanbul). The intervention concept was studied, firstly to determine the effects, secondly the practicability and lastly the feasibility in original form of the weight loss practice. In parallel to this intervention study, the health behavior of overweight and obese individuals was assessed in detail, by means of a standardized questionnaire, in order to provide a broader basis for the target group-specific modification of the construct. In addition, guideline-based expert interviews and the nutrition and exercise protocol were used to answer the open questions and ensure the timeliness of the data. This ensures a complete target group-specific modification of European prevention concept.
Results:
The results of the intervention study, despite difficulties in implementation, show that exercise and nutritional modifications has significant changes in long-term effects in most study-relevant parameters. The mean body weight of the intervention group decreased from 88.2 ± 16.0 to 82.5 ± 16.0 ( - 5.70 kg) or 6.4 %, and BMI from 33.45 ± 3.61 to 31.24 ± 3.73 (-2.21 kg / m2) or 6.4 % and waist
circumference of 103.7 ± 9.6 cm to 92.9 ± 9.6 cm (-10.8 cm) or 10.4 %. Of the 30 volunteers, only 7.62 ± 4.94 participated in the guided exercise units, and 15.25 ± 5.37 subjects in the dietary interventions. After 26 weeks, 77% (23 people) had dropped out of the study.
The results of the other sub-studies used in this work demonstrate that the physical activity and nutritional habits of people living in Turkey, according to the DGE guidelines, are not in tune with a healthy lifestyle. The average daily sitting behavior was over 6.55 hours. The fourth method used to survey the current dietary and physical habits of locals in Turkey and those living in Germany showed no significant differences between the country of origin in terms of diet and physical activity. It also showed that the prevention concept for Turkey could also be used for the Turkish population living in Germany without major changes. Summary and conclusion:
The results of the intervention study and subsequent expert interviews reveal that culture-specific modifications to the framework is required. These modifications should include expectations, interests and preferences of the target group, in the content design, implementation and establishment of the concept. These finding are of significance to people with overweight problems, and contribute to international efforts in the fight against overweight and obesity.
