Abstract

Zusammenfassung: Die vorliegende Arbeit untersuchte die Wirksamkeit eines präventivmedizinisch orientierten Minimalprogramms, basierend auf einem Bewegungstraining und einem unterstützenden Ernährungskonzept, als eine Möglichkeit der nachhaltigen Lebensstiländerung über einen Zeitraum von 62 Wochen. Die Untersuchung erfolgte an 72 gesunden männlichen und weiblichen Probanden. Hiervon stellten sich 36 Männer und 13 Frauen für die Untersuchungsgruppe zur Verfügung (Männer: Alter 45,92 ± 6,5 Jahre, Gewicht 86,87 ± 15,4 kg, Größe 177,15 ± 5,5 cm; Frauen: Alter 44,53 ± 6,1 Jahre, Gewicht 78,83 ± 10,6 kg, Größe 167,21 ± 6,2 cm). Die Vergleichsgruppe bildeten 15 Männer und 8 Frauen (Männer: Alter 42,73 ± 5,6 Jahre, Gewicht 89,20 ± 14,0 kg, Größe 176,60 ± 5,9 cm; Frauen: Alter 42,62 ± 3,9 Jahre, Gewicht 70,38 ± 6,9 kg, Größe 161,37 ± 4,3 cm). Die Probanden wurden einer ärztlichen Eingangsuntersuchung unterzogen. Die Personen waren untrainiert und bewegungsungewohnt. Ein regelmäßiges körperliches Training wurde von keinem der Teilnehmer in den letzten Jahren durchgeführt. Zudem befolgte keiner der Probanden bis zum Untersuchungsbeginn spezielle Ernährungsrichtlinien. Die Untersuchung gliederte sich in drei Messzeitpunkte: am Untersuchungsbeginn (T1), 12 Wochen später bei der Zwischenuntersuchung (T2), und 50 Wochen nach (T1) im Rahmen der Abschlussuntersuchung (T3). Während des ersten Studienabschnitts wurde die Untersuchungsgruppe individuell betreut (T1-T2). Jedem Teilnehmer stand für 12 Wochen ein persönlicher Betreuer informierend und beratend zur Verfügung. Ziel war es, die Probanden zur Umsetzung des Bewegungs- und Ernährungsprogramms zu motivieren. Der Informationsaustausch erfolgte über Telefon, Telefax, elektronische Post (eMail) oder auf dem Briefweg. Folgende Parameter wurden untersucht: Körpergewicht, Bauchumfang, Gesamtcholesterin, LDL - Cholesterin, HDL - Cholesterin, Triglyceride, Glukose sowie systolischer und diastolischer Blutdruck. Den Probanden wurde ein einfaches Bewegungsmangelausgleichprogramm in Form eines Minimalprogramms vorgestellt. Dieses beinhaltete ein herzfrequenzgesteuertes aerobes Ausdauertraining mit einem energetischen Mehrverbrauch von 500 Kilokalorien (2093 kj) pro Woche. Als wesentlichste Befunde ergaben sich nach 62 Wochen in der männlichen Untersuchungsgruppe: Reduktion des Körpergewichts um 4,9 kg auf 81,97 ± 13,81 kg (p < 0,01). Verringerung des Bauchumfangs um 3,58 cm auf 93 ± 6,37 cm (p < 0,01). Verminderung des Gesamtcholesterins um 16,3 mg/dl auf 188,81 ± 29,92 mg/dl (p < 0,01). Reduktion des LDL-Cholesterins um 12,83 mg/dl auf 112,39 ± 25,02 mg/dl (p < 0,01). Steigerung der HDL-Cholesterinwerte um 3,74 mg/dl auf 54,08 ± 11,53 mg/dl (p < 0,01). Verbesserung der Triglyceride um 33,39 mg/dl auf 114,33 ± 48,68 mg/dl (p < 0,01). Verbesserung der Glukosewerte um 3,25 mg/dl auf 82,47 ± 9,42 mg/dl (p = 0,083). Reduktion des systolischen Blutdrucks um 6,67 mmHg auf 130,69 ± 9,79 mmHg (p < 0,01) sowie des diastolischen Blutdrucks um 4,53 mmHg auf 83,39 ± 6,07 mmHg (p < 0,01). Die wichtigsten Ergebnisse der weiblichen Probanden der Untersuchungsgruppe lauteten: Verminderung des Körpergewichts um 2,73 kg auf 76,11 ± 10,94 kg (p < 0,01). Reduktion des Bauchumfangs um 2,31 cm auf 86,46 ± 5,09 cm (p < 0,01). Verringerung der LDL-Konzentration um 16,08 mg/dl auf 108,42 ± 18,29 mg/dl (p < 0,01). Absinken des systolischen Blutdrucks um 4,42 mmHg auf 141,15 ± 12,27 mmHg (p < 0,01). Verminderung des diastolischen Blutdruckwertes um 3,46 mmHg auf 86,92 ± 6,63 mmHg (p < 0,01). Leichte Verbesserung von Gesamtcholesterin (p = 0,456), HDL-Cholesterin (p = 0,852), Triglyceriden (p = 0,607) und Glukose (p = 0,456). Die bedeutsamsten Befunde der Vergleichsgruppe waren bei den Männern: Das LDL-Cholesterin erhöhte sich signifikant um 6 mg/dl (p < 0,05). Tendenziell, aber nicht signifikant erhöhten sich der Bauchumfang (p = 0,153), das Körpergewicht (p = 0,187), das Gesamtcholesterin (p = 0,201), die Triglyceride (p = 0,093) sowie die Glukose (p = 0,129), während das HDL-Cholesterin leicht sank (p = 0,079). Die systolischen (p < 0,05) und diastolischen (p = 0,718) Blutdruckwerte veränderten sich ebenfalls nicht signifikant. Als wesentlichste Befunde ergaben sich bei den weiblichen Teilnehmern der Vergleichsgruppe: Der Bauchumfang erhöhte sich signifikant um 2,43 cm auf 86,81 cm (p < 0,01). Analog dazu stieg das mittlere Körpergewicht ebenfalls signifikant um 2,88 kg auf 73,27 kg an (p < 0,01). Der systolische Blutdruck stieg signifikant um 4,38 mmHg auf durchschnittlich 135,63 mmHg (p < 0,01). Der diastolische Blutdruck (p = 0,260), das Gesamtcholesterin (p = 0,272), das LDL-Cholesterin (p = 0,092) und die Triglyceride (p = 0,607) verzeichneten leichte Anstiege, wobei die HDL-Cholesterinkonzentration (p = 0,852) und die Glukose (p = 0,456) leicht erniedrigt waren. Die Veränderungen zeigen einen tendenziellen Verlauf, waren aber nicht signifikant. Zusammenfassend kann festgestellt werden, dass das untersuchte Minimalprogramm nachhaltig eine Lebensstiländerung unterstützen konnte.

Abstract: The work at hand studies the effectiveness of a minimal programme orientated on preventative medicine, based on an exercise programme and supported by a nutritional concept. This was seen as a possibility to sustain a change in life style for a period of 62 weeks. The study was carried out on 72 healthy male and female subjects. From this 36 men and 13 women were available in the test group (Men: Age: 45,92 ± 6,5 years, Weight: 86,87 ± 15,4 kg, Height: 177,15 ± 5,5 cm; Women: Age: 44,53 ± 6,1 years, Weight: 78,83 ± 10,6 kg, Height: 167,21 ± 6,2 cm). The comparison was made up of 15 men and 8 women (Men: Age: 42,73 ± 5,6 years, Weight: 89,20 ± 14,0 kg, Height: 176,60 ± 5,9 cm; Women: Age: 42,62 ± 3,9 years, Weight: 70,38 ± 6,9 kg, Height: 161,37 ± 4,3 cm). All subjects took part in an initial examination. They were all untrained and not used to exercise. None of the subjects had undertaken a regular training programme over the few last years. Furthermore up until the start of the trial none of the subjects had followed any specific nutritional guidelines. The trial was split into three measuring points: The beginning of the trial (T1), 12 weeks later a test during the trial (T2), and 50 weeks later (T1) as part of the concluding tests (T3). During the first part of the study the test group was coached individually (T1-T2). Every participant had a personal coach for 12 weeks. The aim was to motivate the subjects to follow the exercise and nutrition programme. The communication of information was done by telephone, fax, and e-mail or by post. The following parameters were tested: Abdominal girth, weight, glucose, triglyceride, total cholesterol, HDL - cholesterol, LDL - cholesterol, systolic blood pressure, diastolic blood pressure. The subjects were introduced to a simple exercise deficiency adjustment programme in the form of a minimum programme. This included heart rate controlled aerobic endurance training with an energy efficient consumption of more than 500 kilocalories per week. The most important findings after 62 weeks from the male test subjects of the test group were: A reduction in body weight of 4,9 kg to 81,97 ± 13,81 kg (p < 0,01). A decline of the abdominal girth of 3,58 cm to 93 ± 6,37 cm (p < 0,01). A decline in the total cholesterol of 16,3 mg/dl to 188,81 ± 29,92 mg/dl (p < 0,01). A reduction in the LDL-cholesterol of 12,83 mg/dl to 112,39 ± 25,02 mg/dl (p < 0,01). An increase in the HDL-cholesterol values of 3,74 mg/dl to 54,08 ± 11,53 mg/dl (p < 0,01). An improvement of triglyceride of 33,39 mg/dl to 114,33 ± 48,68 mg/dl (p < 0,01). An improvement of the glucose values of 3,25 mg/dl to 82,47 ± 9,42 mg/dl (p = 0,083). The systolic blood pressure reduced by 6,67 mmHg to 130,69 ± 9,79 mmHg (p < 0,01) as well as the diastolic blood pressure by 4,53 mmHg to 83,39 ± 6,07 mmHg (p < 0,01). The most important findings from the female subjects of the test group were: A reduction in body weight of 2, 73 kg to 76, 11 ± 10, 94 kg. A reduction in the abdominal girth of 2, 31 cm to 86, 46 ± 5, 09 cm. The LDL-concentration was reduced by 15, 08 mg/dl to 108, 42 ± 18, 29 mg/dl. The systolic blood pressure sank by 4,42 mmHg to 141,15 ± 12,27 mmHg, The diastolic blood pressure was also reduced by 3,46 mmHg to 86,92 ± 6,63 mmHg. The total cholesterol, as well as the HDL cholesterol, the triglyceride and the glucose all improved slightly. The most important findings from the male subjects of the comparative group were: The LDL-cholesterol increased significantly by 6 mg/dl. The abdominal girth, weight, total cholesterol, the triglyceride, as well as the glucose tended towards, however insignificantly, a slight increase. Whereas the HDL sank slightly. The systolic and the diastolic blood pressure remained insignificantly hardly changed. In the female subjects of the comparison group, the significant findings were: The abdominal girth increased significantly by 2,43 cm to 86,46 cm. In a similar manner the average weight also significantly increased by 2,88 kg to 73,27 kg. The systolic blood pressure increased by 4,38 mmHg to an average of 135,63 mmHg. The diastolic blood pressure, the total cholesterol, the LDL-cholesterol and the triglyceride showed a slight increase, whereas the HDL-cholesterol concentration and the glucose were slightly brought down. The changes showed a trend in tendencies however of no great significance. In summary it can be concluded that the minimum programme that was researched can effectively support a change in life style.
