The following thesis includes studies on the applicability and feasibility of an exercise training program for children and adolescents with end stage renal disease on hemodialysis. The thesis investigates the central question of whether an exercise training program is applicable and feasible. The studies are considered under high application and form the basis for a first definition of limiting factors in this patient group. A concept with application- and target-group-orientated individual training protocols is developed. In a total of four articles the topic of training on hemodialysis conditions in dialysis-dependent children and adolescents is analyzed. The study designs are a structured review, an exploratory pilot study and an application-oriented review.
The structured review demonstrates that the training control is achieved through targeting the training program on the performance potential of the individual, on the clinical picture, on the subjective perceived exertion, on the general training principles and on normative load. The efficiency and safety of training program can be proved. Detailed training courses or standardized training recommendations aren’t given (Article 1 - structured review). Due to the explorative study the applicability and feasibility of intradialytic training program can be demonstrated. A subjective controlled training program is difficult to implement. A training program at the ventilatory threshold VT1 is possible with stable blood pressure values (Article 2 - Applicability and feasibility). However, a summary of the subjects’ results without considering their medication leads to inconclusive findings. The ability to increase cardiopulmonary performance is multifactorial (Article 3 - Effect Analysis). Recommendations and limiting factors for the practice of sport can be formulated (Article 4 - application-oriented Review).
The implementation of an endurance training programs in children and adolescents during hemodialysis is feasible and safe. The multifactorial field of research can and must be de-veloped and deepened in many ways.
